Executive Summary :

Objective: To expand the associated surgical and prosthetic inventory of the developed dental implants to suit various clinical indications, such as immediate implant placement with loading, full mouth implant retained prosthesis, and computer guided surgeries; Design and test novel micro/nano-surface features to enhance osseointegration and enable early loading of the implants, such as nano-pits, nano-flowers, nano-tubes on a micro surface; Develop related new implant macro-surface design (mini implants, self-tapping implants, bi-cortical implants, implants with varying thread pitch and design) to complete the product portfolio of dental implant system for updation with scientific development; and Clinically test the developed novel implants and its new supporting ancillaries through multiple case series requiring advanced diagnostic and CAD-CAM based surgical planning.

Summary: The goal of modern dentistry is the identification of clinical needs associated with missing or deficient teeth and maxillofacial tissues, and rehabilitation of the function, comfort, appearance, and health of patients using biocompatible substitutes. These substitutes are called dental prosthesis and apart from the conventional methods like complete dentures, removable partial dentures and bridges, dental implants have emerged out as the best treatment modality in lost tooth rehabilitation. The NMITLI Nationally evolved project on “Development of Dental Implants” was targeted towards the development of indigenous implants for affordable healthcare. Wherein, the prototypes were developed in an in-house facility at IIT Delhi and the surface treatment protocols were developed. The second phase led to creation of a regulatory norms compliant test manufacturing facility at IIT Delhi and conduction of a duly approved clinical trial of these implants to establish their safety and efficacy. The clinical project at Maulana Azad institute of Dental Sciences aiming to place the developed implants in consenting patients established the clinical efficacy and safety of the developed device. The study was designed as a prospective randomized controlled non- inferiority clinical trial with a parallel group hybrid design. Analysis of the survival rate of implants after 1 year in function demonstrated 97% survival rate. The procedure of dental implant placement and rehabilitation conventionally takes about 3- 6 months in total. However, newer scientific concepts have enabled faster treatment time in certain complex clinical conditions such as maxillary sinus lift procedures, immediate implant placement in extraction sockets and immediate crown placement (loading), and rehabilitation of deficient alveolar ridges. Hence, there is need for a next generation of implants, which can be placed in clinically challenging situations and can assure osseointegration with reduced treatment times. Such a challenge can be overcome by creating the corresponding R&D and manufacturing processes indigenously. This may become a unique selling proposition of the enhanced catalogue of CSIR-NMITLI dental implants for the rest of the world. Since the technology would be developed indigenously, it will promote the growth of native industry, manufacturing capabilities, and will directly influence the affordability and penetration of the technology.
