Executive Summary :

Myocardial infarction (MI), occurs when the oxygenated blood flow gets impeded or stops in the coronary circulation, resulting in damage to the myocardium. It is the major leading cause of mortality in high to middle-income countries. Myocardial infarction accounts for approximately 30 % morbidities of the total worldwide. The world health organization forecast that by 2030, the rate of MI will increase by 11% (WHO, 2011). In India, the prevalence of MI in individuals of middle age and above 60 years is 50 per 1000 males and 52.3 per 1000 females. Global epidemiological studies estimate that about 40% of MI patients generally have (LVDD) left ventricular diastolic dysfunction. Interestingly, 30%-45% of such patients progress towards heart failure with an initial event of MI followed by post-myocardial infarction remodeling of the myocardium. Pathologically, the myocardium post-MI remodeling process is characterized by increment of the ventricular cavity and excessive fibrosis resulting in deterioration of pumping efficacy. Factors leading to cardiac remodeling and resulting in compromised function are yet to be delineated. Hence it is important to study myocardium extra-cellular matrix (ECM) proteome or Matrisome. Although ECM remodeling sought to be one of the main mechanisms of this progression, there are big lacunae in understanding the ECM remodeling (changes in Matrisome) during myocardial infarction. Using cardiac fibroblasts in-vitro, relevant animal Models in conjunction with high-resolution mass spectrometry-based approaches we plan to dissect the global myocardium ECM Matrisome changes during post-MI remodeling to delineate ECM remodeling.
