Executive Summary :

Autologous chondrocyte implantation is a common surgical intervention that requires expanded harvested chondrocytes from non-load bearing areas, leading to a loss of cell phenotype and poor biomechanical properties. Hydrogel systems have been found to be an effective encapsulating system for chondrocytes, maintaining cell viability and phenotype while supporting neocartilage formation. However, developing these gel structures into a 3D defect model is challenging. A two-component injectable hydrogel system based on chitosan, a'microfracture plus' surgical intervention, has been transferred to a start-up industry. This new hydrogel system offers a better regenerative approach for cartilage repair. The photocrosslinkable gel system, which can be injected with chondrocytes and photocrosslinked after application, offers a neutral pH environment for encapsulating chondrocytes. To maintain chondrogenic redifferentiation with hyaline-like phenotype and biofunctionality, small molecules like Kartogenin, salidroside, Kaempferol, and Honokiol can be conjugated into the gel system through controlled release strategies. The efficacy and biofunctionality of the different small molecule injectable gel systems will be assessed through autologous chondrocyte implantation using the proposed gel systems in a full thickness cartilage defect in a rabbit model. The results will enable clinical validation and attract industries to transfer this technology, leading to commercialization and supporting national efforts to boost the innovation ecosystem in the country.
